Thank you for choosing Dr. Kvitko & Associates. Our primary mission is to deliver the best and most comprehensive dental care available. An important part of this mission is making the cost of optimal care as easy and manageable for our patients by offering several payment options.
For treatment plans of $500 or more, we offer a 5% courtesy accounting adjustment to patients who pay for their treatment with cash or check prior to start of care.
Dr. Kvitko & Associates requires payment prior to the completion of your treatment. If you choose to discontinue care before treatment is complete, you will receive a refund less the cost of care received.
For treatments under $2000, we also offer in-house financing. Payment may be made in thirds. We charge 1.5% interest on all accounts over 45 days.
For larger, more comprehensive treatment plans of $2000 or more, a 20% deposit is required to secure your initial treatment appointment.
For patients with dental insurance, we are happy to file with your carrier to maximize your benefit and directly bill them for reimbursement for your treatment. If we do not receive payment from your insurance carrier within 45 days, you will be responsible for payment of your treatment fees and collection of your benefits directly from your insurance carrier.
Failure to return to the office to have a lab created procedure seated does not absolve the patient from the responsibility of the entire cost of the appliance, crown, fixed or removable prosthesis.
A fee of $100 or 20% is charged for patients who miss or cancel more than 1 time in a calendar year without a 2 business day notice.
Dr. Kvitko & Associates charges $100.00 for returned checks.
Should you fail to pay your account in full, all collections and attorney fees required to collect your account balance and all interest accrued from the date of service remains your responsibility.
